Dear Parents,
Thank you for considering Shepherd of the Hill Preschool for the academic and social needs of your child.
This information is to help you with the registration process and give you a timeline which will be followed as we approach the new school year.
MEDICAL FORM:
- Due to preschool office by opening week and dated less than four months prior to opening week. (September 11, 2008)
- Medical reports are valid for two years for preschool children.
- A medical report indicating that the child has been appropriately
immunized must be on file at the center. Students must be immunized
against chicken pox (Licensing Standard 407.310) or show
proof of immunity.
- Lead screening and TB test. Children aged one to six
years must have either a lead risk assessment or a lead screening
and the age appropriate series of Pneumococcal conjugate vaccine (Prevnar).
A TB skin test is to be included in the initial exam unless waived
by a physician. Medical exemptions made for children should
be indicated by the physician on the child's medical form.(407.310)
- Student medical history. (back of the form) Should be
filled out and signed by the child.s parent. (407.310)
REGISTRATION FORMS AND FEE:
- Forms and fee are due at the time of registration to ensure a place in our program. Class size is limited and fill up quickly.
SUPPLY LISTS AND ORIENTATION MEETINGS:
- You will be sent a letter sometime in July with the dates for Student Orientation and Parent Orientation as well as a list of supplies needed for the school year.
TUITION PAYMENTS:
- Tuition payments are usually due by the 15th of each month beginning with the October tuition. The SEPTEMBER TUITION is due to the office by Tuesday August 15, 2008.
